whilst its not the most tidy example, this is how iv recently done mine...
got a spare cover, drilled/filled/hacked out the centre of the original 'lifter' hole,
chopped up a zrx1100 cover to get the s/cylinder mount and drill through to loosely bolt into position.........
extended the push rod....

had it welded by a local guy, sprayed it black and fitted it all together......
i did try just bolting the slave to the cover but the bolt holes ended up too close to the 'hole' so the mount was used to spread the load, it does push it out a tad but i can live with that
i used zrx11 master cylinder, slave and even the pipe although now im happy that is working ok iv got a bit of braided to fit,
as said, not the neatest but functional........and that'll do for me
