starter clutch

Problem with slipping starter clutch.
If I tighten the bolt to 18lb the clutch slips, if tightened more it engages but the starter motor continues to run which indicates that the gear is not free
Any ideas

Should that have a woodruff key?

Check crank and rotor taper if they have started to wear the thrust washer may be too thick and binding on the gear.
